This form is used to withdraw money from a Collective Investment Account for investment into an ISA. We recently made some changes to this form. These changes are:

• The form now caters for three scenarios outlined on the front page, with deadlines clearly stated, rather than needing a separate form to cater for tax year end requirements.
• We have added more clarification about how we will carry out Bed & ISA instructions.
• Clearer reference is made to the required online ISA application.
